Terms and Conditions: Broker Appointment Upon Claim Resolution

This Agreement ("Agreement") is made and entered into as of [Date] by and between [Your Name/Company Name] ("You") and iRisk Management Limited ("iRisk").

WHEREAS, You may be seeking insurance coverage and require assistance with a potential claim.

WHEREAS, iRisk is a licensed insurance broker offering services to assist clients with insurance procurement and claim resolution.

NOW, THEREFORE, in consideration of the foregoing premises and the mutual covenants contained herein, the parties agree as follows:

1. Appointment of Broker Upon Claim Resolution. By checking the box associated with this Agreement, you appoint iRisk as your insurance broker for all future insurance needs, effective upon the successful resolution of your current insurance claim.
2. This appointment supersedes any existing broker relationship, regardless of whether you have previously appointed another broker.

Scope of Services.

Upon appointment, iRisk agrees to provide the following services:

1. Assist with obtaining quotes for future insurance policies.
2. Advise on coverage options and policy selection.
3. Facilitate policy renewal.
4. Assist with any future claims you may have.

Term and Termination.

This Agreement shall commence upon the successful resolution of your current insurance claim and shall remain in effect for a period of one (1) year, automatically renewing for subsequent one (1) year terms unless terminated earlier by either party upon written notice provided at least thirty (30) days prior to the end of the then-current term.

Your Responsibilities. You agree to:

Entire Agreement.

This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior or contemporaneous communications, representations, or agreements, whether oral or written.

Governing Law.

This Agreement shall be governed by and construed in accordance with the laws of [Your Jurisdiction].

Severability.

If any provision of this Agreement is held to be invalid or unenforceable, such provision shall be struck and the remaining provisions shall remain in full force and effect.
